During interviews conducted on 2/28/2025 staff person # 1 (DOH see LIS code sheet) and staff person #2 (DOH see LIS code sheet) admitted to carrying child #1 (DOB see LIS code sheet) from the Pre-K room, through the school-age room and into the hallway. Staff person #1 stated on 2/17/2025 Child #1 became upset in the Pre-K room and threw a chair, a desk and various toys, all landing near other children. Staff person #1 called the office for assistance and staff person #2 responded. Staff person #2 stated they tried to get child #1 to come with them to the office but child #1 kicked and screamed and refused to walk along with staff person #2. Staff person #2 stated that they attempted to pick up child #1 to carry the child out of the room away from the other children but child #1 continued to kick and scream. Staff person #1 stated that they wanted to help staff person #2 so they picked up child #1's feet while staff person #2 had child #1's arms and they carried the child out of the room, through the school age classroom and into the hallway where they put child #1 down on the floor. Staff person #1 was heard saying " It's sad the school age children have to see you like this" as staff person #1 and staff person #2 were carrying child #1 through the school age room. Staff person #3 assumed responsibility for child #1 once the child was in the hallway and spoke with child #1 to calm them down.

During staff interviews conducted on 2/8 and 2/9/2024 several staff members stated they have witnessed Staff Person #1 pick up rest cots with their hands and/or a foot approximately twelve inches / one foot off the ground and drop it to the floor in order to wake up multiple children at the end of naptime. Staff members interviewed stated this is typically done to Child #1 (DOB see LIS code sheet) as this particular child usually falls asleep 10 minutes prior to nap time being over and this frustrates Staff Person #1.

During staff interviews conducted on 2/8 and 2/9/2024 a couple of staff members stated they witnessed Staff Person #1 grab Child #1 (DOB see LIS code sheet) by the shirt and whip the child around causing the child to fall on the floor. Staff members who were interviewed stated Staff Person #1 did this because he/she was frustrated the child had a bowel movement in their pull-up.

During staff interviews conducted on 2/8 and 2/9/2024 several staff members stated they have heard Staff Person #1 say that if he/she could discipline like in the past that he/she would, while making a spanking motion with their hands. This statement was made to other staff members in a loud voice while in the presence of children. Staff Person #1 admitted to saying this to other staff members. During staff interviews conducted on 2/8 and2/9/2024 several staff members stated they have heard Staff Person #1 call children "dumb" and "stupid" to other staff members in a loud voice while in the presence of children. Two staff members stated they have heard Staff Person #1 call Child #2 "fat" and "fat ass" to other staff members in a loud voice while in the presence of children.

During staff interviews conducted on 2/8 and 2/9/2024 several staff members stated out of seven boys in the preschool classroom Staff Person #1 will make Child #1 (DOB see LIS code sheet) wait until last to get changed during "potty time" even knowing that the child's pull-up is soiled. Staff members stated Staff Person #1 has been heard yelling at the child "Why can't you just go in the potty? This is frustrating!"
