    3270.113(b)/3270.113(d) · A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Tiered LIS 1. Children must be supervised at all times. A facility may not use harsh, demeaning, or abusive language in the presence of children. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of in person training regarding caring for children with difficult behaviors. The legal entity must receive DHS approval for the training. The operator shall provide a date for when this training will be completed.

    Based on interviews with two people from an outside agency during complaint investigation, witness #1 reported to have seen staff #2 forcefully pull a child by the arm. Witness #1 did not know the name of the child. This incident occurred on March 25, 2025. Witness #2 reported to have seen staff #1 grabbing a child by the face and yelled directly at the child. Witness #2 did not know the name of this child. Witness #2 did not recall the exact date in which this was observed.

    During an in-person renewal inspection on 10/25/24, exposed bolts were observed on the outdoor playground. There were two cracked storage bins observed in the basement classroom with pinch points.
