A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.
During renewal inspection, it was discovered that staff #1 lived outside of the state of PA within the last five years. Out of state clearances were obtained by the director the following day so staff was able to resume working, however out of state clearances were not in the file at the time of inspection.

Until such time as the required training has been completed, staff person #1- must be supervised, when interacting with children, by an AGS who has completed the required training related to this citation.
During renewal inspection, it was observed that staff #2 did not obtain a PQAS approved CPR training within 90 days of hire.

A renewal inspection occurred at the facility. A review of the facility's fire system testing log was conducted. Regulation requires the fire system to be test at least every 30 days. The following tests lapsed occurring at least every 30 days: 7/4/22 - 8/2/22, 12/2/22 - 1/6/23, 2/2/23 - 3/10/23.

While at the facility unannounced for the renewal visit on 6/24/22 the inspector observed the files for staff #1 and #2 and observed that staff #1 had a FBI originally dated for 6/26/15 and did not obtain an updated FBI until 7/12/20. Staff #2 had a FBI originally dated for 6/24/15 and did not obtain an updated FBI until 7/12/20. FBI clearances are required every five years and must be completed on or before their original date.
