    During renewal inspection, staff files were reviewed. Staff #1's previous dated Pediatric CPR training was completed on 2/25/23. An updated training was completed until 8/13/25. This date indicates that the staff did not renew their certification on or before the expiration of their last training.

The director must arrange for all facility persons and staff to receive a minimum of three hours of training dealing specifically with safe sleep for infants. The legal entity must receive DHS approval of the training content prior to scheduling the training. This training must be conducted by an approved of resource. Further, this training will not be allowed to count towards the annual 12 hours of training required by regulation. The facility staff will be expected to demonstrate their knowledge and ability to ensure that safe sleep practices of infants is maintained at all times.
