A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. The operator shall provide a date for when this training will be completed.
During renewal inspection, staff files were reviewed. Staff #1's previous child abuse clearance is dated for 4/23/2020. Staff #1 did not obtain a new child abuse clearance until 4/28/2025. Staff #2's previous child abuse clearance is dated for 4/24/2020. An updated child abuse clearance was not obtained until 4/28/2025. These dates indicate that an updated clearance was not obtained within the 60-month requirement.

During renewal inspection, it was observed that staff #1 had a previous child abuse clearance dated for 3/27/2019. An updated child abuse clearance was not obtained until 2/13/25. These dates indicate that staff #1 was still caring for children while the clearance was not updated within the 5-year requirement. It was also observed that Staff #1's previous mandated reporter training was dated for 7/30/2019. An updated mandated reporter training was not obtained until 9/15/24. These dates indicate that the updated training was late and not completed within the 5-year requirement.

During renewal inspection, it was observed that Child #1 did not have an updated child service report on file. Last update on the file was done on 9/19/24. Updates are required to be done every 6 months the child is in care.

A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
It was observed during renewal inspection that Staff #1 had a previous health assessment dated for 8/10/22. Staff #1 did not obtain a new health assessment until 2/25/25. These dates indicate that an updated health assessment was obtained within the 24-month requirement.

A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.
While at the facility unannounced for the renewal inspection on 4/6/23 the inspector observed that staff #2 was required to have a health assessment prior to hire (Please see Inspection Summary Code Sheet for DOH) and did not obtain a health assessment and a TB screening until 8/10/22.
