3270.182(7) · A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.
Complaint investigation was conducted on 10/15/25. Based off staff interviews, it was discovered that a parent was not provided an incident report at pick up regarding their child falling and hitting their head.

During renewal inspection, staff files were reviewed. Staff #1 had a volunteer child abuse clearance on file. Staff #1 had request documentation on two separate dates and email verification of the requests on file, however, was still unable to obtain a complete NSOR clearance prior to working with children. Staff #1 obtained mandated reporter training on 9/23/25. This date indicates that the training was not done and completed within 90 days of hire. (See code sheet for date of hire).

The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in § 3270.124(f) (relating to emergency contact information).
During renewal inspection, child files were reviewed. Child #1 & #2 did not have a child service report on file. Child #3's last service report was dated for 1/23/25. Child #4s last dated service report was on 6/19/23. These dates indicate that service reports were not completed and filed within the 6-month requirement.

The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in §§ 3270.51--3270.55.
During a complaint investigation on 8/8/24, Staff admitted to having 13 children in her group. Director later confirmed all children at the center that day were between 4 and 5 years old.

During the course of the complaint investigation the inspector was able to verify that staff #1 did hold a child by their chin preventing the child from being able to move. This was done, according to staff #1, to make the child look at them while disciplining the child in front of a large group of children in public.

During the course of the complaint investigation the inspector was able to verify that staff #1 left a child behind from a field trip because this child did not have their swimsuit on although time is allotted for children to change. Further staff #1 also has provided candy to some children and not to others while in care.

Until such time as the required training has been completed, staff person #5 must be supervised, when interacting with children, by an AGS, who has completed the required training related to this citation. If there are no staff available to supervise staff person #5, staff person #5 may not work in a child-care position at the facility.
Staff person #5 has not completed approved pre-service pediatric first aid and cpr training within 90 days of their date of hire (see LIS code sheet).

While at the facility unannounced on 10/18/22 for the renewal inspection the inspector observed that staff #2 did not obtain new PA State Police and Child Line clearance prior to their expirations. Staff #3 and staff #6 did not update their Pa Mandated Reporter training prior to the current expiration. Staff #4 was hired with a Volunteer Child Line clearance and not an employment clearance. Staff #5 was hired with a Volunteer PA State Police and Childline clearance. Staff #7 was hired with a Volunteer Child Line clearance. Staff #7 had no proof on file of having submitted for and obtained an NSOR clearance. Staff #4, #5, and #7 have all resided outside of the state in the last five years and do not have their necessary previous state's clearances as is required.
