In the outdoor play space, observed a part of the chain link fence which had broken off and the part of the broken link was facing the inside of the play space and was sharp. This is located when you walk into the play area and make a left and is part of the left fence. Part of the outside play space is covered with artificial grass. The artificial grass is not all one piece and the pieces that were previously secured together were coming loose. Thus, causing a tripping hazard to the children in care.

Observed multiple pairs of shoes in front of the door. Additionally , this same entrance door had an approximate six inch portion of cabinet which was in front of the door.

On 6/22/22, observed staff #4 in the infant room. Observed 3 young toddlers, who were in the supervisory group of staff #4, walk from the infant room into toddler area of the main room where a staff was not physically present. On 6/22/22, throughout the course of the inspection, observed school age children , one at a time, leave the school age area of the main room and walk through the hallway and into the bathroom. Staff #1 stood near the desk area of the main room and was not able to supervise all of the children in her supervisory group as the school age area has tall cubbies which obstructed the view of the rest of the school age children under the supervision of staff #1 while she observed the children who went to the bathroom.
