PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.
The file for staff member #2, hired 5/20/22: included a receipt application for the NSOR dated 6/23/22, indicating staffer #2 had not submitted the necessary documents required under the CPSL to be hired on 5/20/22. Staffer #2's file also included a volunteer PA Child Abuse clearance dated 8/3/21 and under the CPSL a volunteer clearance is unacceptable for employees.

The base boards and ledges of the stairwell which leads down stairs were dusty. In the Fireflies room, the wall near the sink was dirty and discolored. In both the Monkeys and Fireflies, the lids of the plastic hands free trash cans had a layer of dust and debris. In the Seahorses classroom, the air vent in the ceiling had layer of dust and debris. The radiator in the Monkeys classroom was dusty. The shelves and window ceils in the Caterpillars room were dusty.

In the Monkeys' bathroom dust was observed on the shelf and other surfaces were unclean. White metal trash cans with rust spots were observed in the bathrooms of the Seahorses' room and Monkeys' rooms.
