At approximately 1:15pm in Room 2 (Sun Room) one staff and three children (2 infants, and 1 young toddler) were observed. During the onset of supervision and ratio, when staff was asked to identify and name the children whom they were responsible for, Staff person #4 identified and named 2 infants thus leaving the 1 young toddler unaccounted for. When asked who was responsible for the one unnamed child Staff person #4 said Staff person #1, who was out of the room on their break, was responsible for that child. Staff person #3, who was in an adjacent room (Room 1, Living Room), also confirmed that Staff person #1 was responsible for the young toddler. Therefore, it was determined that Staff person #1 was observed to not be physically present with their assigned group of children.
