It was observed on 4/11/2024 in the Older Toddler while Staff Person #1 was changing diapers there were several lapses of supervision between 9:55 - 10:15am. Staff Person #1 was the only staff person in the room with six (6) children. Staff Person #1 was observed to take the children into the bathroom one at a time to either change their diaper or have them use the potty. Anytime that Staff Person #1 changed a diaper their back was turned, and they were completely focused on changing the child's diaper who was on the changing table. Staff Person #1 was not able to see, assess or direct the other five (5) children during that time.
