Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. During a renewal inspection on 10/22/25 Cert rep observed Child 1 in Room 2 with a service provider. Staff 4 who had Child 1 in their assigned group, was present in Room 2 however Staff 4 was not able to see and assess the child and service provider's interactions due to a tall room divider that blocked Staff 4's view.. Child's file did not have any documentation to support that the child was signed out or that parents had given consent for the child to be alone with the service provider while receiving these services. The child's IEP did not note the requirement for the child to be alone with the service provider and not supervised by facility staff.
