Continued Non-Compliance: During the renewal inspection on 5/5/26, there were bottles for multiple children in the refrigerator in the Infant 1 room that were not labeled with the child's name. An acceptable plan of corrections was submitted that stated "A message was sent to families on Wednesday, May 7, 2026, reminding them to label infants' bottles prior to arrival at the center. If bottles arrive unlabeled, teachers have been instructed to ask parents to label the bottles using the supplied Sharpie marker or, if the parent is unable to do so, staff will label the bottles immediately upon receipt using the provided Sharpie to ensure compliance." The correction date was 5/7/26. On 5/26/26, the bottes of child #1, #2, and #3 were not labeled. Going forward, the infant teachers will check all bottles upon arrival each day to ensure they are properly labeled with the infant's name. Additionally, if bottles are not labeled, teachers have been instructed to immediately label the child's name on the bottle using the provided Sharpie marker. Staff will also be reminded of this regulation and procedure during the staff meeting scheduled for May 21, 2026, to ensure ongoing compliance.

A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs. This portion of the plan shall have an immediate correction date. 2.The legal entity must arrange for all facility staff to receive a minimum of 2 hours of PQAS training regarding Behavior Management. The legal shall receive DHS approval of the training content prior to the scheduling of the training. The operator shall provide a date for when this training will be completed. 3. The legal entity must submit a policy to address managing challenging children with difficult behaviors. The legal entity shall submit their policy to the Regional Office for approval. Once approved, the legal entity shall review the policy with each staff person and have them sign off on the policy. This documentation shall be retained in each staff's file. The operator shall provide a date by when this will be completed.

On 2/24 and 2/25/25, staff person #1 was seen on video placing her leg over child #1 and later over child #2 to hold them down on their mats at naptime. Staff person #1 was also seen restraining child #2 by sitting behind her, crossing the child's arms across her body, and wrapping her arms around the child. Staff person #1 stated in an interview that she used both of these techniques to try to get children to sleep at naptime.
