It was observed on video that on 11/13/23 at approximately 12 noon, child #1 was hiding their face in a cubby. Staff person #1 and #2 were both present with the child. When child #1 was asked to get out of the cubby, the child's left eye caught on the hook inside the cubby causing immediate injury in the form of a severely swollen eye, a small cut, and ultimately a ripped tear duck needing same day emergency surgery. At the time of the injury, staff person #1 and #2 could not make a determination on the severity of the child's eye injury due to severe swelling and because child could not open the eye to show staff the condition of the eye itself. Staff person #2 contacted the child's parent but did not call for emergency medical care. Staff were of the understanding that the parent had transportation issues and would need to find a way to the facility in order to seek care for the child. It was observed that emergency medical care was needed due to the extent of the injury, but staff did not accompany the child to the hospital, knowing that the parent could be delayed, and wait for the parent there to assume responsibility for the child's care.
