TIERED LIS: 1. A director must be on site appropriate to the school-age requirements, which is at least 20% of the program's operational hours. This portion of the plan shall have a correction date when a Director will be on site for the required amount of hours 2. The legal entity must submit an updated weekly schedule for the director at this location to the Northeast Regional Office on a weekly basis for a period of 3 months. This schedule shall be submitted by close of business each Friday for the following week. If there are any changes to the schedule, they must be communicated to the Certification Representative within 24 hours of the change. The correction date for this portion of the plan shall be at minimum 3 months from when the plan of correction is submitted to the Regional Office. 3. The facility shall have no instances of non-compliance for a period of 3 months relating to a director being present at the facility for less than 20% of the program's operational hours monitored by at least 2 unannounced visits to the facility. The correction date for this portion of the plan shall be at minimum 3 months from the date the plan of correction is submitted to the Regional Office.

During the Renewal Inspection on 1/6/2026, the director of this facility had called out sick. Therefore, no director was present during the renewal inspection, but was employed by the facility. On 1/30/26, the Department was notified of a new Director being named, and a weekly schedule was provided. On 2/12/2026, a verification visit was made on a day when the new director was scheduled to be present. The director was not present on 2/12/2026. The staff who works daily at this location stated that they have never met the newly named director. As such, the director at this facility has not been present the required percent of operational hours at least since the schedule was provided to the Department on 1/30/2026.
