    During a renewal inspection on 6/1/26 Staff 1 and 3 had more than 12 months between fire safety training dates. Staff 1 had fire safety trainings dated 4/8/25 and 5/11/26, and Staff 3 had fire safety trainings dated 4/8/25 & 5/11/26 in their respective files.

The policy must also include a plan for each staff to be assigned the responsibility for supervision of specific children, including the requirement that the staff person know the names and whereabouts of the children in his assigned group and be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. This policy must address the steps staff and the operator will take in order to maintain both similar and mixed aged ratios and supervision group sizes including but not limited to when someone needs to answer the door of the facility or otherwise leave the childcare space. The policy must include how the facility will ensure appropriate supervision and ratios during transition times and during nap time. The legal entity must receive approval of the policies from the Regional Office. Once approved by the Regional Office, the legal entity must review policies with all staff and have staff sign off that they understand and will follow the approved policies The operator shall provide a correction date for when this portion of the plan will be completed.

    During an allocated unannounced monitoring inspection on 3/26/26 Cert observed Staff 2 was not physically present with the children in their assigned group while in the hallway, and Staff 1 was observed to be alone in the room with 4 older toddlers and 5 preschool aged children. Staff 1 and 2 were present in the toddler/preschool room with 9 children (4 older toddlers and 5 preschool children. Staff 2 left the toddler/preschool room and went to the facility hallway. Staff 2 reported they left the childcare space to answer the facility door. When Cert rep questioned Staff 2 about Staff 1 being alone with the children, Staff 2 then reentered the toddler/preschool room and named and identified 5 children (older toddlers) as being in Staff 2's group. Staff 1 identified 4 children (preschool aged) as being in her group.
