Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1.Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. This portion of the plan shall have an immediate correction date. 2.The legal entity must update the existing supervision policy in order to maintain proper supervision. This policy will ensure that all children are properly assigned to a staff person and supervised at all times. This policy must address supervision during transition times, such as visiting the bathroom and staff transitions. The supervision policy must address that staff must be physically present with their assigned children. The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all staff and have staff sign off that the understand and will follow the supervision policy. The operator shall provide a correction date for when this portion of the plan will be completed.

    During a renewal inspection on 9/12/25 Cert rep observed the following incident in the yellow room: Staff 1 had 3 children in their assigned group and Staff 2 had 3 children in their assigned group. Staff 1 and Staff 2 left the yellow room and both entered the closed bathroom with 1 child from each of their groups(2 total children). The remaining 4 children in Staff 1 and 2's groups were in yellow room with two staff however they were not assigned to these two staff. Staff 1 and Staff 2 were not able to see, hear, direct or assess the other children in their group from within the bathroom, due to the door being closed.

    During a renewal inspection on 9/10/25 Child 2, an older toddler child,(See data sheet for DOB) was missing the following vaccine doses: DTAP/DTP/TD dose 4, Hepatitis A dose 1, MMR dose 1, Varicella 1st dose. At time of inspection there was no documentation on file for child 2 that the child is exempt from these vaccines due to strongly held personal beliefs, religious beliefs, or medical reasons. This is not in accordance with the ACIP recommended schedule of immunizations.

    During the Renewal Inspection on 12/17/2021, one of the wooden pillars in the purple room had sharp edges with the risk of splinters exposed. This pillar also had creases in the wood.
