Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Staff #1 and #2 did not update their fire safety training annually as evidenced by a previous fire safety training dated 11/24/25 and a current dated 1/05/26 for staff #1 and a previous fire safety training dated 1/2/25 and a current dated 1/6/26 for staff #2.

On 7/14/21 during the renewal inspection, a portion of the ceiling in the gross motor room had fallen in and a portion of the rug was wet. There was ceiling tile debris on the floor and a strong musty odor. Additionally, the room was being used for storing strollers and other equipment making it unsafe for large muscle activity.
