The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.
During an unannounced monitoring visiting on 4/17/26 Cert Rep observed infant Child 1 (See LIS code sheet for DOBs) sleeping in a bouncer seat. This sleep position is not recommended by the American Academy of Pediatrics. The child did not have documentation on file from a physician that required the children sleep in this position

During a renewal inspection on 3/6/26 Cert rep observed two children did not have 2 feet of space on at least three sides of the children's cribs while they were in use. The space around the cribs was measured at 1 ft 1 inch, 1 ft 4.5 inches, 5 inches, and 3.5 inches for one child's crib, and 1ft 1 inch, 1 ft 4.5 inch, 1ft 4.5 inches and less than 6 inches for the second child's crib.

3270.113(a)/3270.113(a)(1) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The correction date for this portion of the plan must be immediate. 2.The legal entity must update their written supervision policies to be implemented in the facility. The written policy must ensure that each child is able to be seen, heard, assessed and directed at all times by the staff who is assigned supervision the child. The policy must include how the facility will ensure appropriate supervision during both napping and non-napping hours. The policy must also include a plan for each staff to be assigned the responsibility for supervision of specific children, including the requirement that the staff person know the names and whereabouts of the children in his/her assigned group and be physically present with the children in his/her group on the facility premises and on facility excursions off the facility premises. The policy must also address how these groups will be transitioned from one staff person to another during staffing changes. The updated supervision policies must be submitted to the Northeast Regional Office for approval. Once approved by the Department, the director will ensure that all current employees and any new hires are trained on the policies and procedures and it must be documented in their files. The operator shall provide a correction date for when this portion of the plan will be completed.
During a renewal inspection on 3/5/26 Cert rep observed the following concerns regarding supervision: Staff 1 and Staff 2 were present in the room with 11 children(1 Older toddler, and 10 preschool) during nap time. Staff 1 was sitting on one side of a divider shelf with 1 older toddler. Staff 2 was standing on the other side of a storage shelf with 10 preschool children. Staff 1 identified 1 older toddler and 3 preschool children as being in their assigned group, however due to Staff 1's position, Staff 1 was not able to see or assess the 3 preschool children that were identified to be in their group due to the storage shelf blocking their view of the children. Staff 2 was able to see, hear, hear, direct, and assess the 7 preschool children identified by Staff 2 as being in their assigned group.

Staff 1, 2 and 3 were not in a provisional hire window (see code sheet). Staff 1, 2 and 3 were able to obtain the correct PSP clearance certificates which included dissemination dates on 3/7/25.
