This pre-service training was not completed within 90 days of their date of hire. Additionally, there was no evidence that staff person #1 completed any of the approved CCDBG health and safety courses. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in their assigned group. The staff person shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. TIERED LIS 1. Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in their assigned group. The staff person shall be physically present with the children in their group on the facility premises and on facility excursions off the facility premises. This portion of the plan shall have an immediate correction date. 2. The legal entity must create a policy in order to maintain proper supervision. The supervision policy must include the definition of supervision found in Chapter 3270.4. The policy will ensure that all children are properly assigned to a staff person and supervised at all times. The policy must address procedures for assigning children to a supervision group, procedures for staff transitions, supervision on the playground, and transition times such as visiting the bathroom. The legal entity must receive approval of the policy from the Regional Office. Once approved by the Regional Office, the legal entity must review policy with all staff and have staff sign off that the understand and will follow the supervision policy. The operator shall provide a correction date for when the approved policy will be reviewed with all staff.

    A complaint investigation was conducted on 12/4/2025. During investigation, a child from staff person #1's group was on the other side of a half-wall while the rest of the group was in a different area. Staff person #1 did not know that child was not with the rest of the group until certification representative asked if that child belonged to their group. Staff person #1 could not see, direct, or assess that child.
