    A renewal inspection was conducted on 10/7/2025. At that time, a sample of children's files were reviewed. Immunization summaries on file for child #1 and child #2 do not reflect that children received a second dose of the Hep A Vaccine. First dose of Hep A vaccine for child #1 was administered on 6/14/2022. First dose of Hep A vaccine for child #2 was administered on 2/23/2023. There were no exemption letters on file.     Although the facility does have a fenced-in outdoor play space, it was observed on the day of inspection that a group of children were taken outside to play in the back of the building in the unfenced grassy area next to a large pond. The pond would have been considered accessible to the children playing outside in that space therefore making the body of water a visible hazard.

    3270.151(a)/3270.151(c)(2) · A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

    File for staff person #1, hired 9/7/21, contained documentation of a TB screening conducted on 10/5/17; this screening was completed more than 12 months from the date of hire.
