Child #1 was in care at the facility on Monday, 11/3/2025. During multiple interviews with staff, it was reported that the child was acting normal upon arrival to the facility and was very upset by the end of the day. The child's parent reported that the child was inconsolable when picked up that day. On 11/4/25, the child was taken to the hospital and was diagnosed with a buckle fracture to the left leg. The medical team at the hospital determined that the child could not have caused the injury. The injury would have occurred due to the action of another person not the child. During an interview on 11/5/25, without prompting, staff person #1 immediately named child #1 and stated a concern for a leg injury. Staff person #1 stated this was reported to both the mother of the child and to the person in charge at the facility, which both of them deny.

A young toddler was aggressively trying to take a pacifier from another young toddler. Staff failed to respond when the child needed assistance. Certification staff alerted the staff in the space.
