On 3/13/26, provider self-reported that on 3/12/26 at approx. 4:45pm, Staff #1 enclosed Child #1 in a dark bathroom as a form of punishment, resulting in the child being unsupervised for approximately 30-45 seconds. The child was heard crying inside and exited the bathroom crying.
