3270.102(f)/3270.104(a) · Indoor play equipment for climbing shall be installed or used over a protective surface covering which does not interfere with the stability of the equipment. Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

On 4/30/26, at time of initial inspection, a 4'4" climbing structure including a slide was observed in the front corner of room MS4. The area beneath the unsecured 4'4" platform and slide was not equipped with a protective surface covering extending throughout the fall zones of the structure. Additionally, based on the manufacturer specifications, the slide is not designed for commercial use and has been modified from its intended installation (bottom of slide not embedded), therefore compromising its safety.
