    3270.113(a) · Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. 1) Children must be supervised at all times - implemented 02.25.26 2) Legal entity must ensure that all staff members receive training regarding supervision of children. Certification representative will submit training referral. - implemented 04.09.26 3) Legal entity will utilize a checklist within the school age classrooms at the facility to monitor supervision for a minimum of three months from the date in which the checklist is approved. Supervision monitoring will be conducted on facility site by Regional Office staff over a three-month period.

    On 01.15.26 a complaint investigation was conducted at the facility. It was found that, CH#1 was not supervised by a staff person at all times. Staff Person # 1 left Child # 1 alone on the bus while exiting children out of the vehicle and proceeding into the facility site.
