Based on interviews with staff, the following was confirmed: On 4/7/26 at 10:35 AM, Staff Person #1, #2 and #3 were in the gym preparing to transition 16 preschool children back to the preschool room. Staff Person #1 was supervising 10 preschool children. Staff Person #2 was supervising 6 preschool children. Staff Person #3 was assisting the group. Child #1, who was assigned to Staff Person #1, was left in the gym for approximately one minute when the group transitioned until another staff person observed the child was alone.

During the onset of supervision and ratio when Staff were asked to identify and name the children whom they were responsible for Staff person #2, identified and named five children who were napping and Staff person #3 identified and named the 3 children who were playing at the water table area in the back of the room thus leaving 3 children who were napping unaccounted for. When asked again the same three children were omitted.
