On 3/25/26 at approximately 1:00 PM, Staff Person #10 was supervising 6 children (3 older toddler 3 preschool) and Staff Person #11 was supervising 6 children (3 older toddler 3 preschool) in the Duquesne Room. Staff Person #10 was assisting a child in the attached bathroom. During this time, Staff Person #11 was supervising 8 children on the opposite side of the childcare space and was assisting with the transition to nap, as some children were getting onto their mats while others were running around the room and some were playing in the area. Staff Person #11 was actively directing children to their mats. While doing so, her back was turned to 3 children who were sitting and standing near the dining tables on the other side of the child care space. As a result, for approximately two minutes, these 3 children were not receiving direct supervision.
