It was subsequently verified that Staff Person #3 was no longer employed at the facility on 10.1.25. Also, during the allocated unannounced inspection, Cert Rep requested files for all staff who had been hired since 3.6.25 and the facility neglected to present a file for Staff Person #1. During the inspection on 10.21.25, the file for Staff Person #1 was reviewed. When questioned, Staff Person #6 reported the start date for Staff Person #1 as 9.2.25. However, Cert Rep obtained contradictory evidence that Staff #1 became employed in May 2025. Furthermore, although a pediatric first-aid/ CPR completion certification card was on file for Staff #4, it was later determined that Staff Person #4 was not present for the scheduled training on 2.25.25 and had not completed in-Person Pediatric First Aid and CPR training. It was later learned that Staff Person #5 had been directed by Staff Person #7, a person in a management position, to complete In-Person Pediatric First Aid and CPR training as proxy for Staff Person #4. Therefore, the First Aid and CPR card on file for Staff Person #4 was deemed to be fraudulent.
