At approximately 4:22 p.m., a preschool child was observed leaving a bathroom located around the corner from the Kindergarten room, with the staff person located near the middle of the Kindergarten room where they were unable to see, hear, direct, or assess the child. Then, the child washed their hands at the sink that was located immediately outside of the bathroom where the staff person could not see the child. The child returned to the Kindergarten classroom where staff person #1 was located. The child was alone in what was determined to be unlicensed space without a staff person present.
