During a complaint investigation conducted on 11/1/21 all staff spoken to reported that on 10/26/21, at the end of naptime while a staff person was changing another child's diaper, two older toddlers opened the classroom door and exited their classroom and were without proper adult supervision for approximately two minutes until another staff person found the two children in the hallway and were promptly returned to their classroom.
