In the Infant 2 Room at approximately 11:10am four infants were observed to be sleeping in the crib area and two staff were sitting in the rocking chairs near the crib area with their backs to those infants who were sleeping in the cribs. Due to staffs positioning in the room it was determined they could not adequately supervise all the infants who were sleeping in the cribs. In the Infant 3 Room at approximately 11:25am two infants were observed to be sleeping in the crib area, and depending on where staff were positioned in the room, they could not adequately supervise those two infants who were sleeping in the cribs due to the plexiglass room divider having a frosted mosaic privacy screen decal preventing staff from adequately supervise all the infants who were sleeping in the cribs. In the PreK Room at approximately 1:23pm, depending on where staff were positioned in the room, they could not adequately supervise one of the napping children due to where the child's mat was placed, and furniture obstructing the Staff's ability to see and assess that child who was napping.
