Upon entering the older toddler room at approximately 9:40 am on 8/19/21, staff person #3 was observed in the middle of the classroom with 4 older toddlers. As the staff person identified the 4 children, a child's voice could be heard coming from the bathroom. In the bathroom, 2 older toddlers were observed unsupervised in each of the two stalls. Staff person #3 then went into the bathroom briefly, leaving the other 4 children unsupervised in the room.
