3270.113(e) · A facility person may not restrain a child by using bonds, ties or straps to restrict a child's movement or by enclosing the child in a confined space, closet or locked room. The prohibition against restraining a child does not apply to the use of adaptive equipment prescribed for a child with special needs.

Based on a self-report made by the facility, on 12/22/25 staff #1 told child #1, a school aged child, that if the child did not sit down the child would be taped to their chair. Staff #1 reportedly made the statement jokingly to the child to encourage the child to comply with the request. Staff #1 took a piece of masking tape approximately 18 inches in length and attached it to the back of the chair the child was sitting in, then lightly draped it around the child's shoulder and attached the other end to the seat bottom. The tape came loose as soon as child #1 moved their arms.

On 6/7/24 a large crack was observed at the bottom of the green slide, attached to the climbing structure, in the outdoor play space. A piece of caution tape was around the top portion of the slide. The cracked plastic was accessible to children playing in the space.
