3270.27(e) · The operator shall provide to the parent of each enrolled child a letter explaining the emergency procedures. The operator shall also provide to the parent of each enrolled child a letter explaining any subsequent update to the plan.

The letter to the parents explaining the emergency procedures does not address annual emergency drills, lockdown, or accommodations for infants, toddlers, children with disabilities and children with chronic medical conditions.

3270.31(e)(4)(ii) · Staff persons shall participate, at least annually, in fire safety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

The staff file for Staff person #1 was observed to lack documentation of fire safety training for 2024 the two most recent fire safety trainings on file are dated 2/5/23 and 5/2/25. The staff file for Staff person #2 was observed to contain fire safety trainings that were conducted more than 12 months apart; those fire safety trainings are dated 5/8/24 and 6/4/25.

3270.32(a)/3270.192(4) · The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

The staff file for Staff person #1 contained a current State Police clearance dated 1/8/25 which was not renewed within 60 months of the previously dated 12/31/19 State Police clearance expiring. The staff file for Staff person #2 contained a current National Sex Offender Registry (NSOR) check dated 6/26/25 which was not renewed within 60 months of the previously dated 6/8/20 NSOR check expiring.

3270.76 · Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

In the Starbright Room, two ceiling tiles were observed to not be kept clean as evidence by having water damage spots from an unknown water source. In the Stairway off of the PreK Room, 7 ceiling tiles were observed to not be kept clean as evidence by having water damage spots from an unknown water source.

3270.82(h) · A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

The following toilets were observed to lack a handwashing sign: three toilets in the restroom located to the left of the office, two toilets in the restroom between the Toddler Room and Young Teddy Bear Room, and the toilet in restroom in the PreK Room.
