Both items included instructions to keep out of the reach of children. On 12-3-21 it was observed that there was a container of "Home Defense Bug Spray" and an aerosol can of disinfectant accessible to children on the floor in the kitchen. In the kitchen there was a large plastic snack container, a small empty lidded plastic container and a plastic disposable cup observed on the facility's electric stove. On 12-3-21 it was observed that there were multiple dirty dishes stacked high in the kitchen sink. It was observed that there were multiple combustible plastic items being stored on top of the electric stove top in the kitchen. It was observed that there were multiple boxes full of items unsteadily stacked one on top of the other throughout the child care room causing a hazard. On 12/6/21The following hazards were observed to be in the play space on 12-6-21: there was an exposed bolt on the gate of the fence to the left of the facility, there were 2 rusty grills next to the gate to the left of the house, there was a rusty piece of metal underneath one of the grills by the gate to the left of the house, there were rolls of roofing nails on the ground in the rear of the play space by the garage, the fencing surrounding one of the garden areas in the rear of the play space near the garage had the sharp end of a screw protruding from the wooden base and one of the wooden support poles holding up the fencing was detached causing the pole to lean into the play space, there was standing water in a black plastic bin next to one of the garden areas in the rear of the play space, there were 3 pieces of down spout on the ground causing sharp metal edges to be accessible to the children, there was a small piece of cracked plastic on the ground causing sharp edges to be accessible to the children, there were multiple pieces of splintered wood accessible to children on the ground, there was a green bucket with cracked edges causing sharp edges to be accessible to the children, there were deteriorating roof shingles in multiple places on the ground in the outside play space. On 12-6-21 it was observed that there was a brown substance on the wall coming from the ceiling where there was also brown substance. On 12-6-21, it was observed that there were toy bins stacked three high on top of a toy shelf in one area of the childcare room and cardboard boxes stacked three high on top of a different toy shelf in the childcare room; causing the hazard that the toys bins and boxes could fall on the children.

At approximately 1:45 pm, the same child was placed back in the crib for approximately 10 minutes. Throughout the afternoon of the inspection, staff person #2 were observed holding the same child on their lap or having the child stand in front of them. The child was whining and crying and trying to move away from the staff.
