Cert Rep asked Staff #1 if the child had been there in the Side Room the whole time that the Cert Rep was present. Staff #1 stated that the child went to the playground with the group, and when they returned, that he was present alone and unsupervised in the Side Room. Staff #1 admitted to forgetting to name the child when Cert Rep arrived. Therefore, ratio was 0:1 from the time Cert Rep arrived at the facility until Staff #2 arrived and took the two additional children to the Side Room. At approximately 11:09 AM, a school ager was permitted to go use the restroom, located in the hallway outside the childcare space, by themselves alone and unsupervised. From the time the child was permitted to leave the childcare space and until she returned to the classroom, the ratio was 0:1. At approximately 11:23 AM a Preschool Age child was permitted to use the restroom, located in the hallway outside the childcare space, by themselves alone and unsupervised. From the time the child was permitted to leave the childcare space until he returned to the classroom, ratio was 0:1.

    At approximately 10:38 AM, Staff #1 was observed alone in the Front Room with 9 children: including, 1 infant, 3 Young Toddlers, 1 Older Toddler, 2 Preschoolers, and 2 School Agers. Cer Rep later observed that there was also a School Aged child that was in the Side Room alone and unsupervised. At approximately 10:45 AM and then again at 10:47 AM, Staff #1 left the Front Room in order to take snacks to the children in the Side Room, leaving 5 children alone and unsupervised including 1 Infant, 3 Young Toddlers, and 1 Older Toddler. During the time the Staff #1 went back and forth between the two classrooms, the ratio was 0:5. At approximately 11:17AM, a Preschool Age child wandered out of the Side Classroom, even after redirection, putting Staff #1 in the Front Classroom with 6 children: 1 Infant, 3 Young Toddlers, 1 Older Toddler, and 1 Preschooler. During the time the Pre-K Child was present in the front classroom. the ratio was 1:6.

When asked about the long delay in answering the door, Staff #1 stated that they were outside on the housing development's playground, which is not part of the facility premises. After further questioning, it is determined that Staff #1 had been alone with the 10 children on an excursion.
