At the same time, staff person #6, the director, was observed standing in the front hallway and staff person #3 was observed moving between the breakfast room and the hallway to attend to the children in those two separate spaces. When in the hallway, staff person #3 was not physically present in the same space with the 5 children in the breakfast room. Based on conversation with staff person #6, who was still wearing their coat, they had just arrived at the facility and had run late due to a traffic accident. Based on conversation with staff person #3, they were responsible for all 7 children, placing them out of ratio.

On the 1st shelf was a large jug of Chlorox bleach. On a shelf above the sink in the older toddler room, there was a can of Lysol disinfectant spray and a spray bottle of bleach water, which were accessible to some of the 2 year-olds in the room at the time. In the preschool room, there was a tub of Endust screen and electronic wipes under the staff person's desk, making it accessible to the preschoolers in care. All items included instructions to keep out of the reach of children.

Based on conversation with staff person #3, they were responsible for all 7 children, placing them out of ratio.
