    During the incident investigation, certification representative conducted a facility walkthrough and interviewed several staff and found the core root of the fire was preventable. The fire was caused by an unsafe condition (flammable material - cardboard box on a heat source - stovetop) in which staff #1 and staff #2 put the cardboard box on the stovetop.
