At the time of the renewal inspection, ceiling tiles in the bathroom were observed to be in disrepair. Weeds were present on the playground area, along with trash, a saw, and other tools. Broken ceramic tiles, pieces of broken furniture, and damaged fixtures were also observed under the wooden stand and around the corner of the playground. In addition, sections of the green artificial grass covering had become displaced, leaving portions of the play space uneven and visibly hazardous. The door in the main room leading to the water heater was observed to be unlocked, making the water heater accessible to children. Non-compliance since 5/5/26.
