Certification representative was infomed that children had not been in care at the location for over one month. The floors of the location on both levels had various trash, some in bags, others; for example a moldy rug, was rolled up and on the landing of the staircase. There was some construction that had happened that was not cleaned up.

The floors of the facility were in need of cleaning there was a significant layer of dust and grime. The walls of the facility had some sections with molding falling away from the wall and exposing the sharp side of staples. The duct for the heating and cooling system had a 1/4 inch layer of dust and appeared to be in need of service.
