On February 07, 2025, during the initial inspection, there was white substance on the floor in the front childcare and the floor appeared to be visibly in need of cleaning. There was damaged ceiling tile in the back childcare space adjacent to the bathroom that was stained. The back outdoor play space was in need of repair. There was mud and rocks and debris in the space. A large broken toy was present in the back outdoor space.
