During the renewal inspection, certification representative conducted a facility walk through and staff #4 was working alone and allowing children to walk down the hall alone to the bathroom. At some point a school district employee came to tell staff #4 to only allow two children in the bathroom at a time due to a plumbing issue with one of the stalls. Children had to walk past rm 214, which is the custodian room, that was open with various cleaning supplies accessible. Also, two children were released for the day and allowed to get on the elevator, unattended,without proper staff supervision. They both got on the elevator together to leave for the day.
