On April 20, 2023, during the initial inspection, dust was present on the windowsill in Rm #1 under the air conditioner as well as on the other windowsills located in that room. In Rm #4 ceiling tiles were in disrepair over and near the teacher's desk. Room #5- dust and dirt was present on multiple surfaces, pipes were protruding from the floor in two corners of the room. Outdoor play area presented visible trash on the play yard and around the parameters of the fence, four tree stumps were protruding out of the ground towards the back of the yard, sticks/branches were sticking out from the gate on the left side of the yard, green turf was torn and partially lifted posing a tripping hazard.

On April 20, 2023, during the initial inspection, the little blue children's chairs in Room #5 were dirty, multiple large toys' surfaces were dirty and dusty, a white wicker toy crib with a pink sheet had dirt inside of it, all of the outdoor play equipment was heavily dirty and a bolt from the turf was laid out. Branches and dirt were present on the floor of many of the little plastic houses outside. One of the little houses was full of multiple piled up toys inside of it.
