The footage of a nap time in Toddler room showed at least 14 children, ranging in age from young toddlers to preschoolers, with staff person #1. One child, identifiable by a red shirt, was seen leaving the room without supervision. Older school age child #4 was walking in the facility without being supervised. Additional footage from the office area on 4/25/2025 showed Child #1 and Child #2 leaving the office area without staff supervision. The staff present in the office could not see, hear, or assess the children's movements. Furthermore, during the complaint investigation, it was reported that Child #3 did exited the building unsupervised and went to the facility's outdoor gated play space without being seen by Staff Person #1 or Staff Person #2. Both staff were unaware of Child #3's whereabouts until they observed the child outside attempting to squeeze between the poles of the playground gate.
