On 10/17/2023, during the complaint investigation, Child #1 was observed walking around in the preschool area and attempted to open the door to go into the infant classroom area. During this time, Staff person #1 and #3 were fully inside of the infant classroom and Staff person #2 was engaged in an activity with the preschool class while their back was turned. Therefore, resulting in the Child #1 not being in direct sight of any staff member and left unsupervised. Child #1 was allowed to enter into the facility office unsupervised, while Staff person #1, #2 and #3 were all present in the classroom spaces. On 10/18/2023, during an unannounced follow-up visit to the facility, Child #1 was again observed leaving the classroom and entered into the facility kitchen area unsupervised. On 10/18/2023, Child #2 was observed using the back bathroom by the preschool classroom with the door fully closed and out of direct sight of Staff. On 10/18/2023 during a follow up visit to the facility, upon entering the preschool classroom, Individual #3 was observed sitting alone in the front classroom with 9 preschoolers during lunch. Staff person #2 was fully in the kitchen. Staff person #1 was in the office with the door closed and Staff person #3 was in the infant classroom. Staff person #2 acknowledged that Individual # 3 was not a staff person. Therefore, resulting in the preschool children being left unsupervised.
