Observed 8/6/2024 in the outdoor play space there was debris and dirt that was under the artificial grass that was placed above the space. There were bricks closer to the fence where the artificial grass did not cover the entire space. The ground was unleveled when stepping on the surface there were holes in the ground leaving the space not free from any visible hazards.

The door in the middle that separated the room was damaged and not in good repair. Observed in bathroom 2 near the toilet there was a cardboard box that when touched there was exposed wall crumbling of rocks and dirt behind it that were accessible to children.
