TIERED LIS: 1.Children must be supervised at all times. 2.The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. A facility person may not use any form of physical punishment, including spanking a child.
On 6/24/2026 an follow up inspection was conducted to observe corrections of inspection on 5/20/2026. Staff #1 was observed on the floor during nap transitioning and proceeded to hit child #1 on their right arm and back then aggressively grabbed the child and placed their body on to their cot. Staff # 2 had their back turned from children not able to hear, see or access children's whereabouts.
