During the renewal inspection on 06/18/24, Cert Rep observed written staff evaluations in the file for staff persons #1 and #2, dated 05/15/23, staff person #3, dated 04/17/23, staff person #4 dated, 03/11/22, and staff person #5, dated 06/17/22. Cert Rep did not observe any written evaluations for staff persons after said dates.
