Several areas in the facility were observed to be in disrepair. Those areas are as follows: the toilet in the upstairs bathroom with the bathtub did not flush,the floor vent in the infant classroom on the 1st floor is rusty, the floor vent in the classroom straight back from the main entrance on the 1st floor is rusty, and the light in the green bathroom does not turn on. Several areas in the facility were not free from visible hazards. Those areas are as follows: the classroom to the right of the main staircase had a tall toy shelf not anchored to the wall, loose screws on top of the air conditioning unit, a folding table leaning up against the wall. In the dark green room located on the 1st floor there was a wrench on the ground, a loose floor tile, a group of cable wires hanging down from the ceiling, and an upside-down folding table on the floor. The outdoor play space was unclean, in disrepair, and included visible hazards. Overall, the outdoor area was covered in moss, there were piles of old leaves, loose nails, and cigarette butts on the ground. The fence surrounding the air conditioner was had a large opening, there is a hole in the concrete, and nails sticking out of the ramp. There was a stack of wood with splinters located near the fence. The hose was unraveled. There were chairs and pipes leaning against the building. There was sharp metal holding up the white piping. There were several branches wedged in the fencing.
