-During the Initial Inspection on 12/7/22, In bathrooms 1 and 2 on the lower level and in the bathrooms 3 and 4 located on the upper levels had dust on each of the ventilation covers. - There was an orange long cord that was hanging from the ceiling that was used for the projector in the large child space area. - In the walk-in closet area located in the play space, there were numerous protruding nails that were sticking out and accessible to children. - -There were stained ceiling tiles near main entrance by the windows, in the Play Area space near the back closet area and in the Lower Level first bathroom. - The rug at the beginning of the ramp area was torn. - There was dirt and debris located on both window ledges near entrance of the facility. - Near the front window in large child space area, there was a loose landline box with loose wires that is accessible to children. -In plays pace area there was a loose black box by the tables. -In the lower-level bathroom there was a loose fire alarm that was hanging off of wall. -Tables, chairs, a water cooler, and two large storage bins were present in the play space area which created an unsafe space for gross motor activity.
