3270.106(a)/3270.119 · Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child. Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

On 11/8/2022 Certification Representative observed that 2 infants and one young toddler were left sleeping in equipment that is not appropriate rest equipment from 11:28 a.m. to 11:47 a.m. A young toddler was asleep in a high chair, one infant was asleep in a bouncy seat and another infant was asleep in a swing. Children were not placed in the appropriate rest equipment and in the appropriate sleep position until Certification Representative asked staff to move the children to proper rest equipment.

On 9/2/2022 facility child #1 was not properly supervised when being dropped of at school. Facility staff #1 failed to bring the child into the school building and sign the child in with the appropriate responsible person. Facility staff #1 let child walk into the building and enter the school while a contractor was working on the front door and did not properly escort the child into the building ensuring proper supervision until in the care of the school.
